You can nominate a patient for a smile makeover. Her’s how!

Do you know someone who has been through a unique hardship or cannot afford these procedures and who is deserving of a smile makeover? If so, please submit a short video explaining their hardship and why they are deserved of a smile makeover by including how it would improve their life. For more information about the application process please give us a call so they might have an opportunity for a smile makeover.
